I am assuming since you are looking at a 5th wheel trailer to gooseneck hitch adapter that the trailer is a 5th wheel. For the greatest amount of offset, 8 inches, go with the Ranch Hitch, # AM3139. It is also rated the highest of any of the similar adapters; 24,000 pounds gross trailer weight and 6,000 pounds pin weight. You will also need the safety chains, # AM3109.
To calculate how much clearance you will have, measure from the center of the gooseneck ball to the back of the truck cab and add 8 inches. Then measure the width of the 5th wheel trailer overhang and divide by 2 to get half. Take the first measurement plus the 8 inches, and subtract the second measurement to get the amount of clearance the offset adapter will provide. You will need at least 4 inches to safely make 90 degree turns.
The 6 inch lift could pose a problem. The Ranch hitch is among the shortest adapters at 11-3/4 inches tall in its shortest position. To determine the height you will need, park the truck and trailer on level ground. Back the truck up as if you where connecting to the trailer. Measure from the top of the ball to the bottom of the pin box (not the king pin). This distance is the height you will need.
