I have a Equalizer model 90-00-0400 hitch rated to 4,000 pounds. Is this the correct sway bracket jacket? Thanks,

Since you have the 4,000 lb Equalizer system # EQ37041ET the kit you need is the # EQ95-01-5540. What you mentioned fits the higher rated systems.
