I have a solution for you, but unfortunately, there's not an exact item for what you are looking for.

Prior to purchasing a hitch extender, I recommend measuring your clearance. You'll simply measure from the center of the hitch pin hole on your hitch to the furthest point in a straight line away from the hitch. Any extender you use will need to cover at least this distance to be useable.

If you are at exactly 18 inches, then I recommend the Tow Ready Hitch Extender for 2 inch Hitches, part # HE18. This is a straight line, J-pin compatible hitch extender that will push your load 18 inches away from the hitch. This extender will reduce your overall tongue weight capacity by 50%.

If you're at less than 18 inches, but more than 12-1/2 inches, I recommend the Brophy Hitch Extender with Step, part # ST18. This will add a convenient step to your extender, allowing access to the camper. Additionally, the extra dead space created by the step will allow you to better open the door. This extender will reduce your hitch's weight capacity by one third.

You do have an all in one option, however it does not have a swing-away feature. The Hollywood Racks Sport Rider with Cargo Carrier, part # HR1450Z-85 includes a two bike carrier plus 100 pound capacity cargo carrier. Weighing in at around 95 pounds, its fairly hefty but will allow you to use the tilt-away feature to access the camper. With the hitch extender plus step, this can come close to simulating the swing-away feature you're after.

The cargo carrier and bike rack can be used independently of each other, and include bike and hitch locks to keep your bikes safe.
