Yes, Curt Hitch # C15845 is designed to fit Ford F450 Cab and Chassis models from 1999 - 2017 (as of October 2017). I checked with my contact at Curt and he verified that the Fontaine Classic Traveler Bed should not interfere with the hitch installation. This hitch fits vehicles with a frame that is 34" wide from outside to outside. So, you'll need to double check this measurement prior to ordering.
In reference to your second question, yes, you should replace your hitch after a rear-end collision. The hitch could be compromised during a collision making it unsafe for use. Small cracks may not be visible to the naked eye and this could negatively affect the weight bearing capacity of the hitch. You should also have the frame members at the rear of the truck examined by a body shop. They will take measurements to verify that nothing has been knocked out of alignment or bent. Any damage to the frame may cause the hitch mounting points to be out of alignment and may also negatively affect the hitch capacity.
