Yes, the Reese Sliding Fifth Wheel Hitch # RP30075, and the Reese Universal Base Rails and Installation Kit, part # RP30035 will fit your 2004 Ford F-350.
For a rail kit I would strongly recommend you get a Custom Reese Fifth Wheel Installation Kit, part # RP50082-58. This kit is designed to work specifically on your 2004 Ford F-350 and will cut installation time in half and require less drilling.
I attached installation instructions and a video of this product being installed on a similar F-350.
