Adding a 5th wheel hitch to your 2012 Mitsubishi Fuso fe125 will require a custom installation. I recommend the Reese Low-Profile 5th Wheel Trailer Hitch - Dual Jaw - 30,000 lbs # RP30054 that will have 4 holes in the side brackets to your flat bed. The center to center mounting holes are 15 inches apart front to back and side to side are 25 inches. You may need to reinforce these areas to make them strong enough for the towing capacity of this hitch which may include adding a brace or even welding a cross member depending on your specific application.
As far as placement of the hitch itself, you will want to center this hitch on your rear axle or place it an inch or two in front of it but not behind it.
