Do you have any hidden hitches for a 2012 highlander that allows for a full size spare?

For your 2012 Toyota Highlander with full size spare tire, I recommend the Draw-Tite Max-E-Loader Trailer Hitch Receiver - Custom Fit - Class III - 2" # 75726. If you look at the page for this product you will see in the pictures that a full size spare tire is a fit with this hitch installed. Hidden hitches have been discontinued as a brand.
If you will be using this hitch for towing, I recommend the Tekonsha T-One Vehicle Wiring Harness with 4-Pole Flat Trailer Connector # 118449 for your wiring. This will give you a 4-way flat trailer connector. The connection is behind the driver's side interior trim panel in your rear cargo area. The Modulite plugs into factory wiring harness under your dash on the driver's side. You will need to located the "towing fuse" in your fuse box and insert the included fuse. Your owners manual will show you where.
For a ball mount, I recommend the B&W Tow & Stow 2-Ball Mount - 2" Hitch - 5" Drop, 5-1/2" Rise - 10K - Black # BWTS10037B. This is an adjustable ball mount to allow you to tow level. It has two hitch ball size options and a really nice stowable feature to give you a clean look and protect your shins when not in use.
