Realistically there is only one cargo carrier we can recommend for your 2012 Lexus RX 450h to give you the ability to fully open the rear door of your vehicle with the carrier installed. We have a video installation of the Draw-Tite Class III Max-Frame Trailer Hitch, item # 75153, on a 2007 Lexus RX350, see link, that shows the distance from the center of the hitch pin hole to the farthest out part of the rear bumper at 1-1/2 inches, this leaves the receiver portion of the hitch out about 1/2 inch past the rear bumper.

The pin box you came from would have the same dimensions as a Lippert 1621 pin box. The distance from the rear mounting holes to the center of the king pin should be 25-5/8 inches. That would also be the distance for a replacement pin box like part # LC328492.

Since the Inno roof rack (parts hyperlinked below) can be mounted at varying locations on the flush rails of your 2018 Lexus GX 460 I recommend going with that system. It has a lower weight capacity at 140 lbs (as opposed to 165 lbs from Yakima) but in my experience most people don't plan on max-ing out the weight capacity of their roof rack so the benefit of adjustability will out-weigh (no pun intended) the higher weight capacity of the Yakima system.
