Installing the frame bolts will depend on the fifth wheel installation kit that you use for your 2004 Ford F-150. Since you referenced the Reese M5 Series Fifth Wheel Hitch, # RP30921, you will need the Reese Elite Series Under-Bed Installation Kit, # RP30064, for your 2004 Ford F-150. This kit includes all of the parts and hardware necessary to complete the installation.
Since the frame of your F-150 is enclosed, you will have to run the carriage bolts through the frame using the fishwire technique. First, you will remove the jounce bumper and use that hole to run the included fish wire through the frame. Thread the wire onto the bolt and slide the spacer block on from the opposite end of the wire. Then you will fish the wire through the jounce bumper hole to the pre-existing mounting holes on the outboard side of the frame.
Page 7 and 8 of the installation instructions explain how to install the bolts through the frame. I have attached a link to the installation instructions for you to check out. I have also attached a video we made on the fishwire technique for you to check out. The application in the video is different from your application, but it should give you an idea on how to properly fishwire the bolts.
