I recommend checking out the Kodiak Disc Brake Kit # K2HR526D for your application. This is designed for 5.2K - 6K axles and fits applications that use inner bearing # 25580, outer bearing # 15123, and a grease seal with a 2.250" inner diameter. I recommend verifying this is what you have just to be sure.
If your trailer isn't already setup for a hydraulic braking system then you need to install the HydraStar Electric Over Hydraulic Actuator Kit # HBA16-252-82 as well. This gives you the electric-over-hydraulic actuator so your brake controller is compatible with the hydraulic brake system. It also has all of the brake lines necessary to complete the installation.
Attached are some review videos for you to check out.
