I spoke to Lippert about the 300638 and 028-000222523 numbers provided in your picture, and they confirmed that you do have a 1621 pin box. With that said, the Reese Goose Box # RP85FR will be a perfect fit for you. All mounting hardware is included with this and it will be a direct bolt on application.
