I do have an option for you, but there is not a custom fit trailer hitch receiver option for your 1991 International 4700 with a low profile chassis.

Based on the picture you provided your truck appears to be pretty low to the ground and the 16" tall brackets the Draw-Tite Universal Hitch # 41990-16 has would more than likely put the hitch too low.

Instead the Draw-Tite # 41990-07 would be a better option as it only has 7" tall mounting brackets. The brackets can be trimmed to get the correct length you need for the hitch to sit exactly how you want.

This hitch will need to be welded to the frame of your truck, and I recommend having a qualified welder do the welding to make sure that the hitch will be strong and able to tow a trailer without any chances of breaking or failure.
