The numbers you provided really aren't an identifier for the lens or the lamp they came from so unfortunately it won't help much in identifying a replacement lens, as they're just numbers that signify that the lens meets DOT standards. However, the photo was pretty helpful (thanks for sending it in!)

To help conclusively match the lamp to the proper replacement lens, I'd need to know the following:

-Dimensions of Lens
-Center to Center Spacing of Screw Holes
-Any Identifying Numbers From Housing of Lamp

If the lens you have measures 2 x 2-3/4 of an inch and the screw holes are 2-5/16 inches apart, center to center, the # 55-15R is almost certainly a match.

If your lens is a tiny bit larger, at 2-1/16 x 2-7/8 inches with a 2-5/16 inch center to center hole spacing, go with the # A9RB.

These are the most likely matches, but if your lens measures differently see of you can find a part number on the housing of the lamp and get back to me with that.
