how does this adaptor work if i am also running brakes through my brake lights?

In standard U.S. 4-pole wiring the brake LIGHT signal is combined with the turn signals for both sides of the trailer. This is shown in the linked photo of a 4-pole connector. The actual braking power signal in a trailer wiring harness (that engages the electric brakes) is never combined with any other function.
The Hopkins 4-Pole to 5-Pole adapter part # 47515 is intended to allow adding a circuit that carries the vehicle reverse light signal which will operate an electric reverse lockout solenoid in a trailer coupler like # T43397007K with solenoid # T4748800. This reverse light signal prevents the coupler from engaging the brakes when the vehicle and trailer back up.
