I am not sure what you mean by "extended tongue" but if you can send me a photo of your trailer, I would be happy to see if and how a weight distribution system would be affected by this feature. The Curt TruTrack Weight Distribution System part # C17501 is designed to be used in applications in which the total tongue weight (TTW) of the truck/trailer is between 1,000 lbs - 1,500 lbs. TTW is calculated by adding the loaded ready to tow tongue (TW) weight of the trailer by any cargo stored behind the rear axle of the tow vehicle. Any easy way to find the TW of the trailer is with a TW scale like part # e99044.
The TW of a trailer is generally between 10% -15% of the trailer's gross trailer weight (GTW). Therefore, your trailer could have a tongue weight ranging from 1,200 lbs to 1,800 lbs if your trailer did indeed have a GTW of 12,000 lbs like you stated. If this is the case, then the above system would likely be underrated. I recommend using the Blue Ox SwayPro Weight Distribution w/ Sway Control part # BXW2000 which is rated for a TTW between 1,500 lbs to 2,000 lbs. This system works with a 2-1/2" trailer hitch receiver therefore you would also need to upgrade the trailer hitch for use with this system. If you can tell me the year, make, and model of your vehicle I would be happy to recommend one for you.
I have also attached a review video of the Blue OX system for you as well.
