Is the ball pre-installed in the head and properly torqued?

The Equal-i-zer weight distribution systems like # EQ37121ET come with the hitch ball needed, but do not come pre-installed. You will need to use a thin walled socket like # ALL643216 or # EQ70-00-4800 to torque it to manufacturer specs.
