Need diagram of plate req. to mount 30054 hitch. Pls send bolt and hole pattern

According to my contact at Reese, their Low-Profile 5th Wheel Trailer Hitch # RP30054 has a base that measures 17- x 28-inches. The four mounting holes' centers are spaced at 15-inches front-to-back and 25-inches left-to-right.
Do note that welding and possibly also some fabrication will be required when you mount this hitch to the truck's frame. The required under-bed steel plate will need to be a bit close to the size of the 17 x 28 mounting base.
