I do have a few options for you to consider however, there is no jack that matches the exact description you have provided. The highest rated weight capacity we have for a bolt-on swivel jack is 2,000 lbs. For this, I recommend the Fulton F2 Swing-Up Jack w/ Pivoting Footplate part # F1413230134 as this will match all but one of your needs mentioned. It has a maximum lift height of 10", features a swivel design for easy storage, and bolts onto the frame without the need for welding.
Most jack stands are rated at a much higher capacity than they can actually hold/lift however, if you are certain that you will need 3,500 lbs of lift capacity then you will have to forgo the swivel feature by using the Fulton Marine Jack - 5K - Square Tube, Fixed Mount part # HD50000101. The Fulton 5K marine jack has a maximum weight capacity of 5,000 lbs and will provide up to 14" of lift. Compared to similar jacks, it sits much higher on the frame rail to account for ground clearance issues. Additionally, the Fulton 5K features a drop leg that will reduce the amount of rotations needed to raise the jack.
