More than likely the # C45CR was slightly bent during shipping. This is usually the reason the holes won't line up during installation. Unless it is severely bent (cracks or chipped paint can indicate it is unsafe) you can bend it back and get your hitch installed. You can use a vise, step, or even some boards to bend it back. We have never found any factory wiring on the rear of a 2012 Toyota Tundra that came from the factory with a 4-way flat.
