I recommend using the Curt A25 # C16036 which will fit in the factory pucks on your 2015 Ford F-450 since this doesn't include the wiring that you don't need and will give you a nice hitch at a good price point. The A25 hitch heads are fully articulating, which make for easier hookup and they also do a good job of cushioning road shock.

I'll start off by saying that in almost all cases, using a hitch extender will reduce your tongue weight by 50%, so your 350-lbs TW/3500-lbs towing capacity hitch is likely limited to 175-lbs TW and 1750-lbs towing. We have a couple options that would work with your B&W Companion 20K Hitch # BWRVK3300. If you're only wanting to cover the head you can use the Reese 5th Wheel Trailer Hitch Cover # RP30055. If you're wanting a cover that drapes over the hitch, I recommend the Classic Accessories Cover # CA80116. This cover measures 24 inches x 24 inches x 18 inches.
