Are Factory Tow Hooks Removed for Trailer Hitch Installation on a 2012 Lexus RX350
Question:
Do you remove and reinstall the two tow brackets under the rear of the vehicle or do you install the hitch without them. ?

The instructions which I have linked for you do not mention the removal of the factory tow hooks if present. However, I have seen pictures of the factory tow hooks and you will need to remove then, install the hitch, and if you wanted reinstall the tow hooks when securing the hitch.
If you plan on doing some towing I recommend using the etrailer ball mount kit # 989900. It comes with 2 ball mounts, 2 balls, a hitch pin with clip, and a carrying bag. For trailer wiring use # 118478.
