The mounting holes on the Draw Tite hitch part # DT45507 that you referenced are separated by 40 inches from left to right and the hitch opening will be 1-1/2 inches below the frame surface the hitch attaches to.

Best option would be the Draw Tite Class V Service Body Hitch Tube part # 41990 which has the capacity you need and is long enough for your application. This is just a hitch tube though and your best option would be to find a welder/fabricator who can come up with a bracket solution to place this hitch where you'd need it on your frame.

Mainly due to the unknown factor of how it's going to attach to the trailer frame. The crosstube itself is more than strong enough but it depends on the brackets used which there's no easy way to give specs on what it would take to work properly.  The typical bracket that get used with this hitch are the part # 4907 which don't quite run as far forward on the frame as the brackets that normally are weight distribution approved.
