Expert Reply:
We do have a couple of trailer hitches that fit the 1987 Dodge full-size vans. From Draw-Tite we have # 75140 rated for 500 pounds tongue weight and 5,000 pounds gross trailer weight. And from Curt we have # C13041 rated for 600 pounds tongue weight and 6,000 pounds gross trailer weight.
Both of these are Class III trailer hitches with 2 inch receivers. Current pricing is available on the product pages.
The hitches will definitely fit the frame of the van but if the ambulance body hangs down further than the regular body there can be some fit issues. And if the body is longer (but doesn't hang down any further than the standard van body) then it could just be harder to reach the receiver.
