I recommend tracing the wiring from the 4-pole connector back to see if it plugs into a connector or if it's hardwired to your tail lights before purchasing anything. If the plug was located along the drivers side frame rail on your 2018 Chevy Express Van, maybe taped up or rolled up inside the frame rail with 7 pins. It could have a cap on it to protect the pins, it is most likely the tow package wiring. I would test the plug first. While I do have a possible solution for you, there aren't any custom fit front mount hitches designed for your 2019 Chevy 4500 Cutaway Chassis so for your application you'll need to use an adjustable hitch like the Curt # C13701. This will require you to trim the crossbeam of the Curt hitch as well as figure out how to fit it to your Chevy so if this isn't something you're comfortable with I recommend having a shop help you out with it.
