Even on a trailer from the 1960's I've never heard of a 5 on 4 bolt pattern. Where exactly are you measuring the 4 inches? With a 5 lug pattern you need to measure from the center of one bolt hole to the outside edge of one of the furthest bolt holes from that one. Just like is shown in the picture. I've also attached a help article on measuring your bolt pattern and a template you can use if you'd like.

We don't have a database that tells us what parts are on specific trailers, but I was able to find some photos of Shorelander boat trailers around that age and it looks like something like the Fulton Single Axle Trailer Fender with Top and Side Steps - White Plastic - 12" Wheels - Qty 1 # F008572 would work for you. This fender is 7-1/2" Wide x 33-1/2" long x 8-3/4" tall and works with trailer wheels up to 12" diameter.

It sounds like maybe you need something like the Curt Weld-On 2" Receiver for Custom Fabricated Hitches # E17, which could be welded to the tube you're referring to and would give you a secondary 2" receiver. This receiver does have a 500-lbs tongue weight, but it should be noted that any time you extend a receiver away from the cross tube more than 9" that the tongue weight is reduced by approximately 50%.

You'll need a high/low adapter to bring the tow bar within 3 inches +/- of level. The adapter you need will depend on the height difference between the base plates and the receiver on the motorhome/tow vehicle.
Roadmaster offers several different adapters:

I have a solution for you, but you will not be able to use the base plates from your 2012 Chevy Colorado on your 2019 Jeep Renegade Trailhawk; base plates are designed to be vehicle specific, so they won't be able to be transferred between vehicles. Additionally, from what I was able to determine in the owner's manual, the Jeep Renegade Trailhawk cannot be flat towed.

I have a solution for you, but the Draw-Tite # 76290 does not come with wiring; this is fairly commonplace since not everyone will need a trailer wiring harness with their hitch.
