I sent all of your info to Lippert and they told me that your axle uses standard 3,500 lb hub/drums with a 5 on 4-1/2 inch bolt pattern. So the hub you'd want is the # AKHD-545-35-EZ-K that you referenced and then for brake assemblies you'd need the part # AKEBRK-35-SA which is a pair of left and right assemblies that are self adjusting. The brackets you have are an older style of Draw Tite brackets which aren't currently supported. So that means is you'd need a new system that includes new brackets. Which even if your brackets were still supported by the time you'd piece together a kit with everything but the brackets you'd be at or above cost of getting a new system.
