Changing a trailer over from hydraulic drum brakes to hydraulic disc brakes requires both the new brake and hub/rotor assemblies that use your same bearing set, a new actuator, brake flanges and a brake line kit. The pressure rating of a drum brake actuator is lower than that of a disc brake type, generally 1000-1100 psi versus 1600 psi for a disc brake actuator. This is why the actuator needs to be changed. You will also need to select new hub/rotor assemblies that match the bearings used on your current spindles and your wheel bolt pattern. The article linked at right will give you all the background on this upgrade process as well as all items required.

The article includes a chart that lists the typical components used in integrated hub-and-rotor hydraulic disc brake applications, depending on the axle weight rating and diameter.
