For your Promaster, I recommend checking out the:
-Fan-Tastic Vent Roof Vent w/ 12V Fan and Remote # FV807350.
This fan extends only 4-1/2" above the roof of your trailer when closed, a variable speed, reversible fan with 2 modes of operation, a rain sensor, and a remote control.
This fan appears to hit all the wickets that you're looking for, and will work great with your Promaster van.
Attached is a product overview video for you to check out as well.
