In order to meet your measurement requirements you are going to need a universal hitch like the Draw-Tite Ultra Frame Service Body Trailer Hitch Receiver # 41990-16. This hitch will need to be welded on for installation.
The hitch fits frames up to 44" wide and uses a 2" receiver. It also has 16" tall welding brackets which can be modified to give you the desired height. These brackets are long enough to allow you go get the 12" distance from the bottom of the frame to the top of the receiver you are looking for.
