Will this fit on a 2020 Toyota Highlander? Do the crossbars have to be 29 7/8inch apart from outside to outside or is that inside the bar to inside

Crossbar spread is normally measured from center to center of the crossbars themselves. The SportRack Vista XL Cargo Box # SR7018 can fit a crossbar spread of 23-5/8", 25-7/8", 27-7/8", or 29-7/8" so you'll definitely be able to get it to fit on your 2020 Toyota Highlander.
