What size rise do I need to get an 18 inch heigth from ground to middle of the 2 inch ball? This is recommended from U haul.

In order to choose the correct ball mount height for a Toyota Rav4 you simply need to measure from the ground to the top of the inside edge of the receiver tube and then from the ground to the bottom of the coupler on the trailer while level. This will be different for every vehicle and trailer combo so the best option to ensure you can tow level and with multiple trailers if you ever wish to switch would be to go with the etrailer Ball Mount Kit for 2" Hitches part # 989900 since it has multiple hitch height options.
