What Does Derale Series 8000 Transmission Cooler D13503 Include Insta-Dapt Fitting Instead Of Barb?

Why does this kit have a fitting and not just a barb?

The Derale Series 8000 Plate-Fin Transmission Cooler Kit w/Barb Inlets # D13503 includes the insta-dapt as shown in your picture due to Derale finding that a larger portion of vehicles have a 1/2"-20 thread so that is what the put in their kit.
If you can use the adapter, you will install it into the transmission cooler port on your radiator as shown in the included diagram, or if you do not have that size port you will just install the cooler bypassing the factory cooler as shown in the included diagram.
