If you already have a Blue Ox tow bar, then I suggest the Blue Ox Base Plate Kit - Removable Arms, # BX1139. See product review and installation video and verify this fitment against your style bumper. These base plates should not only fit Fit Blue Ox tow bars but also Aventa, Aladdin, Alpha, Adventurer, Ambassador, Acclaim, Avail, Allure, and Trion tow bars as well and many Roadmaster tow bars too with a sold separate adapter, # RM-031 or #RM-031-5.
Since you have already flat towed and have the needed set-up for your Equinox, I will assume the tow bar has all the safety cables etc.. I also assume you are familiar with the need to tow the vehicle with the bar fairly level, using a high/low adapter, so I attached a link to those items for selection after the needed height measurement is determined.
The Roadmaster 4-Diode Universal Wiring Kit for Towed Vehicles - 7-Wire to 6-Wire Straight Cord, # RM-152-98146-7 is a confirmed fit to your 2021 Jeep. Installation video is attached showing how to wire to a 2020 Jeep Wrangler.
You state you already have the Blue Ox Patriot Portable Braking System, # BLU37TR. The Second Vehicle Breakaway Kit for Blue Ox Patriot Supplemental Braking Systems, is # BRK2505. For just the Breakaway Cable replacement itself, # BX2940931.
