We have the RV Bathroom Faucet - Dual Knob Handle, # 277-000042 that has an overall height of 5" and meets your request of being a dual handle with inlets at 4" on center, this however does not have a rotating spout. Typically a 4" on center inlet is for bathroom fixtures, to have one with a rotating spout is going to be taller than your requested spec of 5" max. Please review the links to other options.
