I have another option for you that has great reviews and is similar to the hitch you mentioned, but we do not currently carry Putnam products.
I recommend taking a look at the B&W Turnoverball Underbed Gooseneck Hitch W/ Custom Installation Kit # BWGNRK1110 if your 1994 Ford F-250/350 has overload springs, or # BWGNRK1100 if your truck does NOT have overload springs.
The B&W Turnoverball offers a 30000lb gross towing weight limit, 7500lb vertical load limit, and has a limited lifetime warranty. The Putnam Hawkeye 15091 you were looking at is rated for 27000lbs, and requires angle iron to be used to installed on your truck. Since Putnam has closed, finding replacement parts in the future would also be an issue.
The best part about the Turnoverball is it allows you to store the ball upside down when you do not need the ball which will keep the bed of your truck clear, and allows you to use the B&W Companion Gooseneck-to-5th-Wheel Trailer Hitch Adapter # BWRVK3500 to tow a 5th wheel trailer when needed without needing the above bed rails that get in the way when the hitch is removed.
The B&W Companion # BWRVK3500 offers a 20000lb towing capacity, uses dual 1" thick jaws that offer 360 degree of king pin contact for a safe and secure connection, and the fully articulating head allows for easy hitching to your trailer even on un-even ground.
I included videos for each of the products mentioned for you to take a look at.
