To install the B&W Turnoverball Underbed Gooseneck Trailer Hitch # BWGNRK1300 in the bed of your 1992 Dodge D250 Pickup you will need to align the hitch between the truck bed and the frame of the truck and then you will drill through the hitch to the frame of your truck and then run bolts through the holes you drilled and secure them on the other side. You don't need to lift off the bed, but you will need to possibly use a lift to get access to the area on the frame and under the bed in order to install it.
There is not a bracket kit available for this particular model so it will need to be connected directly to the frame. I've also linked the instructions for this hitch to this answer for you to have a look at.
