The only wheel and tire combinations we offer for the ST145R12 size and 4 on 4 inch bolt pattern you listed are on aluminum wheels and are not load range E. Check out the Kenda KR25 ST145R12 Radial Trailer Tire and Wheel # AM31208 and the Kenda KR25 Radial Trailer Tire and Wheel # AM31208HWTB.
The Kenda Tire # AM10140 that you referenced is a load rang E, but we do not have a 4 on 4 inch wheel with the same capacity as most 4 on 4 inch hubs have capacities near 2,000 lbs.
While these wheels and tires have not been balanced with weights they have been lined up so that the heaviest part of the wheel has been lined up with lightest part of the tire so that they are as balanced as they can be without weights. Typically this setup works for people, but you might also want to have the tires balanced at a wheel shop.
