To install the Bulldog Gooseneck Coupler Head Repair Kit, # BD1542S01, start by removing any of the old parts. Installing the new parts is fairly straight forward. Start by placing the spring on the threaded end of the shaft.

Next, put the handle on with sticker facing away from the shaft. Make sure that the shaft flat spot and hand point out in the same direction. The handle can be tricky and you may need to persuade it on with a mallet.

Next, slide this assembly through the head on the hand opening side. The short end of the spring fits in the groove under the head. The hook end goes around the handle. Work the handle and spring assembly into place.

Then, make sure the assembly is sticking out all the way on the other side so you can reinstall the the small retention washer. On the other end tighten the nuts onto the shaft one at a time. Test the handle operation to make sure everything moves smoothly and that the spring pulls the handle back into place.

I made a short video that shows me installing the kit that should also help.
