We don't have an already assembled adapter for what you are looking for, but you can pretty easily make one with components that we sell.
For the 5-way round for your tow vehicle you would want the part # PK11501, for jacketed wire you would want the part # DW04923-1 which is sold by the foot. Then for a 4-way flat connector that would connect to the 5-way on your trailer you would want the part # HM48030.
