If you have the OEM puck system on your 2019 Chevy Silverado 2500 HD then the only way to get an offset for your gooseneck trailer is by swapping out the coupler with something like the B&W Extend-A-Goose Gooseneck Coupler # BWTEXA4200 as there aren't aren't offset gooseneck balls like the Curt # C602 you referenced for the pucks.
On the slight chance that you don't have the OEM puck system on your pickup I recommend installing the B&W Turnoverball # BWGNRK1016 so that you can use the 4" Offset Ball # BWGNXA4085.
