I reached out to my contact at Husky who said that the Replacement Upper Assembly part HT87638 for Husky Brute HB4500 series electric trailer jacks has recently been discontinued which is why you are having trouble finding it.
Since this is the case you will need to replace your whole electric jack. I recommend replacing it with the Stromberg Carlson Electric Trailer Jack # JET-5000 which is rated for 5,000 lbs just like your current jack. Unfortunately replacing your jack is your only option at this point.
