We may have a solution that will work for you, but the 5" Offset Ball for Hide-A-Goose Gooseneck Trailer Hitches # 19308 does not fit the factory gooseneck on your 2018 GMC Sierra 2500HD. Instead, your best option is to use the B&W Extend-A-Goose Gooseneck Coupler # BWTEXA4200; this coupler fits standard 4" diameter outer tubes, works with 2-5/16" gooseneck balls and will give you 10" of offset.
