As far as a straight axle goes we do not have one that is an exact match for the measurements you have provided. If you wanted a straight axle it would have to be a custom order from a local place. What I recommend instead is to go with a Timbren Axle-Less system which can be used in a tandem set up.
The Timbren systems will also replace the suspension with rubber springs that will hold up and function better compared to leaf springs. They are available from capacities of 1,200 pounds to 7,000 pounds per set. I have included a link to the options for you. If you can tell me the diameter of your existing axles I can estimate the capacity. The bearing numbers in the hub are also useful in determining axle capacity. And there may be a tag on the axle or capacity information stamped into it as well.
