The hydraulic trailer brake you referenced, Item # AKUBRK-35L, fits 4-bolt flanges with a 2-7/8-inch hole spacing and is suitable for use on a 3500-lb axle. It fits the driver's side and brake mounting bolts are included. For the passenger side, use Item # AKUBRK-35R.
Nuts and lock washers are sold separately and are available in brake mounting hardware kit Item # BIK-00 (one kit needed per wheel).
I recommend the Brake Adjustment Spoon Item # W80630 as well.
