Most likely what you need is the part # AKHD-545-35-K as this matches the things you listed. This is a 3,500 lb hub/drum with a 5 on 4-1/2 inch bolt pattern. This fits the standard 3,500 lb axle spindle.

You'll want to measure the spindle on your trailer to verify this. If the bearing surfaces are 1.378 for the inner diameter and then 1.063 for the outer bearing surface this would be what you need.

Or if the bearing numbers are L68149 and L44649 that is the other way to verify this.
