How does this hook up to the trailer ball? Any pics?

The trailer ball would bolt to the head of the weight distribution system you're using, and the weight distribution head would bolt to the shank, as shown in the photo I've provided.
If you weren't needing weight distribution and were only wanting to get the trailer ball at the proper height to tow the trailer level, you'd be better off using a ball mount to achieve the proper height. The FAQ article I'll link for you shows exactly how to determine the ball mount rise or drop needed.
If you'll be towing several different trailers, an adjustable ball mount might be your best bet.

Make sure you choose the correctly sized trailer ball for your trailer coupler, and that the capacity of the ball and the ball mount exceed the fully loaded weight of your trailer. You'll also want to ensure that the trailer ball you choose has the same diameter as the ball hole of the ball mount you choose.
