Your 2003 Ford F-350 has industry standard above the bed rails. The mounting slots side to side on center should measure 10 inches for the inner set, 20-1/2 for the center set, and 29 inches for the outer set. Front to back they are approximately 22 inches apart on center.
I recommend the Curt gooseneck plate for 5th wheel rails # 16055. This adapter is rated for 25,000 pounds gross trailer weight and can be mounted with the ball further back or forward depending on the amount of clearance needed to make turns. It fits in the inner set of slots spaces at 10 inches apart.
