The Lock N Roll Articulating Hitch # 336VS503505 can do a rise/drop of 4-1/4 inches but there isn't a way to allow this to also have a dual ball mount design on it like you asked about. So to cover that you'd need a different adjustable ball mount like the part # AM3411 which has a similar rise/drop but has the two ball sizes you need as well.
