No, it doesn't but I have a less expensive one that does. For your 2023 Mazda CX-9 you will want the Draw-Tite Hitch item # 76020. This hitch is custom fit and bolts to your Mazda with included hardware.

I really like this hitch because it mounts behind the rear bumper cover leaving only the 2" receiver visible. It also has a built in tab that makes it easy to connect a wiring harness using the Mounting Bracket 4-Pole Flat item # 18144.

If you are planning on towing you will also need a wiring harness item # 118278 and a ball mount kit item # C45534.

The underbody panel you're referring to can be placed back on, but you'll have to trim it around the etrailer Trailer Hitch Receiver - Custom Fit #e87AR in order to get it to fit. It is a bit challenging with this vehicle so most people would…

The etrailer hitch part #e87AR that you referenced actually comes with 4 bolt leaders so they wouldn't need to be supplied. The bolts are 1/2 inch though so for a replacement bolt leader you'd want the part #C58400.
