I took a look at what we have for you and it appears that you have the Electric Jack w/ Footplate # EJ-3520-WBX.
With that being said, the lens is actually not offered as a replacement part, so you're going to need to replace the entire housing with the:
-Replacement Cover for etrailer and Ram Electric A-Frame Jacks # EJ-3520-CVRW.
This product will come with the lens you need, and will give your RAM jack a fresh new look!
